I have been wondering the same thing lately, especially on a recent trip to Chicago (not that it is particularly more pavement-ugly than other large cities). But its also prevalent in numerous cities of Brazil, which you can't make the "old world" argument for, because some of the most famous installations there have been close to mid last-century. It certainly makes wide pedestrian areas much more attractive.
